Disney does a good job of providing assistance to Guests with Disabilities of many different types. I have a few suggestions I hope will assist you.
The parks are pretty big so if you are unable to walk long distances or stand for longer periods of time, you may want to consider renting a wheelchair or ECV while you're in the parks. This may help you conserve your energy and make your visit more enjoyable.
Disney offers Wheelchair and ECV Rentals for use at all 4 theme parks (Magic Kingdom, Epcot, Hollywood Studios and Animal Kingdom). Reservations are not accepted and wheelchairs and ECVs are available on a first come, first serve basis. They must remain in the parks in which they were rented. If you visit other parks on the same day, you'd just have to show your receipt at the next park to obtain another one. I recently provided another Guest with information regarding wheelchair rentals that you may also find helpful.
A lot of the queues now are covered or indoors so that should help provide you some relief from the sun. We always take our small umbrellas with us. In Central Florida, you typically get lots of sun or rain and an umbrella protects from both.
Be sure to take advantage of Disney's FASTPASS Service that will reduce the wait times for attractions.
Visit Guest Relations as you enter a theme park and explain your situation and see what current options they have available to assist you.
